Notes | The reactivity of the antiserum is directed to the hidden determinants of Ig lambda light chain. In immunoelectrophoresis this antiserum is only reacting with Bence Jones proteins as well as free light chains of the lambda type. No reaction is obtained with intact polyclonal or monoclonal immunoglobulins. To detect and identify free light chain type and Bence Jones protein in serum or urine by immunoelectrophoresis or double radial immunodiffusion. It is of great importance as an aid in the diagnosis of B cell malignancies such a multiple myeloma and non-Hodgkin lymphoma, and in monitoring their therapy. In immunoelectrophoresis use 1.2 μl of sample in a well with a diameter of 1 mm against 60 μl antiserum in a 20 mm long channel. Electrophoresis for 1 hour, 20 mA. The detection limit of this technique in serum is about 25 mg/l; in the case 100 times concentrated urine is used the detection limit can be brought down to 0.5 mg/l. The fact that in about 40% of the case the Bence Jones protein was only detectable in either serum or urine emphasizes the necessity to investigate both fluids. |
